What to bring
Please ensure you bring the following to your appointment:
- A referral from your practitioner, GP, Specialist or other healthcare professional
- Medicare Card
- Healthcare Card (if applicable)
- Pension Card (if applicable)
- Department of Veteran Affairs card (if applicable)
- Any relevant previous images
